The Lifetime Care & Support Authority would like to be advised of a person's severe injuries as soon as possible after the motor accident using the Severe Injury Advice Form

The Severe Injury Advice Form includes a medical certificate that needs to be completed by the treating doctor. This is to confirm that the injuries are likely to meet the eligibility criteria for entry to the Scheme. The Authority asks that health professionals assist people with this process when required.
If the circumstances of the accident and the injuries appear to meet the eligibility requirements for the Scheme the Authority will appoint a Lifetime Care and Support Authority Coordinator (LTCS Coordinator) who will assist the injured person to apply to the Scheme.
A coordinator may also be appointed to meet with the injured person to find out more information about the circumstances of the accident and to liaise with the treating health team, about their injuries.
