Your LTCS coordinator is your main contact person at the Authority.

They will work with you to plan your return to school, leisure activities or work and will coordinate your injury-related services.

The LTCS coordinator may contact you while you are in hospital to provide you with information about the Scheme and how to apply.

If your application is accepted, the LTCS coordinator will continue to help you and your family to:

• understand what services are available and how they work
• help you to get the most from the service you receive and get back into your usual activities
• organise required care and other services.
