Treatment, rehabilitation and care services can only be paid for by the Lifetime Care & Support Authority when the person has been accepted as a participant of the Scheme. To be accepted into the Scheme you must complete an Application Form.
The Application Form includes a medical certificate that needs to be completed by the treating doctor. This is to provide information about the severity of your injuries in relation to the injury criteria for entry to the Scheme.
You or your guardian must sign the statutory declaration on the application form in front of a justice of the peace.
The Authority may need to gather further information, including information from the police and investigate the accident before a decision can be made.
After receiving your application, the Authority will write to you to advise whether you have been accepted into the Scheme as an interim participant. An interim participant is a person who has been accepted into the Scheme on a temporary basis while their injuries are still improving.
As an interim participant you will have the cost of your treatment, rehabilitation and care paid for up to two years. After two years you will be assessed for lifetime participation. If you are eligible for lifetime participation you will receive the treatment, rehabilitation and care you need for the rest of your life.
If you do not agree with the Lifetime Care & Support Authority's decision about eligibility for the Scheme you can dispute that decision. Refer to the Dispute Resolution section on this website for further information.
If you were injured in a motor accident that was caused by the fault of another driver, you may be eligible for compensation under the Compulsory Third Party (CTP) insurance scheme and a claim should be made against the insurer of the driver at fault. The Application Form for participation in the Lifetime Care & Support Scheme is separate to a Personal Injury Claim for CTP insurance.
