Forms to request services

All requests for services and equipment for a participant in the Scheme need to be submitted to the Authority on the appropriate form.

Once complete, the form must be submitted electronically to requests@lifetimecare.nsw.gov.au

Pre-accident Participant Information Form

Use this form to report the participant's pre-accident status. This form must be submitted to the Authority prior to, or with, the Discharge Services Notification Form.


Discharge Services Notification Form

Use this form to notify the Authority of the services the participant requires on discharge.


Community Living Plan

Use this form to develop a Community Living Plan for a participant in the LTCS Scheme.


Assessment and Service Requests

Use this form to request assessments required before a Community Living Plan can be developed.

Use this form to request services outside of those approved in the Community Living Plan.

Contact the participant's LTCS coordinator prior to submitting this form.


Care Needs

The Authority has developed new procedures for reporting the care needs of participants and requesting attendant care services. These new forms are being piloted from 4 October 2011 to 31 January 2012.

The Care Needs Assessment form will not be accepted from 1 November 2011.

Service providers should use this form to document the participant's overall reasonable and necessary care need.

Service providers should use this form to request the attendant care services the participant would like to receive. This request can only be submitted with a Care Needs Report.

This document contains answers to some of the most frequently asked questions for attendant care within the Scheme.

The Care and Needs Scale (CANS) must be completed when undertaking a care needs assessment for adult participants with a brain injury.


Equipment

The Authority's equipment procedures have been developed in partnership with EnableNSW as part of the Common Equipment Prescriber Guidelines Project.

If you need to request a prosthetic limb for a participant in the Scheme, please contact David Gonzalez

Use this form to request equipment, aids or appliances (excluding continence or disposable products) for all participants of the Lifetime Care and Support Scheme.

The prescriber should indicate when requesting equipment if the evaluation is required. Evaluation should be completed following a minimum of 4 weeks of use in the participant's home or community setting.

The evaluation form may also be used to report an unforseen event with the prescribed equipment.

Use this form to prescribe and request continence equipment and ongoing consumables for a participant of the LTCS Scheme. The instructions contain information about how to complete the form, as well as who can prescribe continence products in the Lifetime Care & Support Scheme.

The EasyOrder scheme is an initiative introduced by the Authority to enable selected treating clinicians to order inexpensive items of equipment for participants without first completing an Equipment Request Form. Lifetime Care & Support Coordinators are able to order these items on behalf of clinicians who do not have access to the EasyOrder Scheme. For more information about the EasyOrder scheme, please speak with a coordinator or with Dianne Croker

Equipment Loan Pool - MOTOmed

The Authority has purchased a MOTOmed for use by participants on a 'loan' basis. Participants wishing to access this item must have a current treating physiotherapist to assist with the set up, programme design and outcome measurement of the loan.

To be eligible to loan the MOTOmed, participants must understand and sign the Participant Agreement form and submit this form along with an Equipment Request Form completed by their treating physiotherapist.

Education Support

Use this form to request education support services for a participant in the LTCS Scheme.


Travel

Use this form to request travel and accommodation bookings for a Participant in the LTCS Scheme or Service Provider.
