Case Management in the Lifetime Care & Support Scheme

In 2010 the Authority published Case Management in the Lifetime care and Support Scheme  (136 kb)

The document was drafted after extensive consultation with key stakeholders and forms the basis of forums run by the Authority on case management in the Scheme. The purpose of the document is to articulate the models under which case management is currently delivered to participants of the Scheme, and as such will be up-dated periodically. It also outlines the Authority's expectations of case managers and quality service delivery.
